Middlewich Supported Living

Our Middlewich Supported Living Service provides 9 self-contained apartments for adults who want to live as independently as possible while having access to flexible, person-centred support.

Designed to promote independence, the service offers a safe and welcoming environment where individuals can develop everyday living skills, build confidence, and work towards achieving their personal goals. Each apartment is equipped with assistive technology infrastructure, helping people maintain their independence while ensuring support is available whenever it is needed.

Our dedicated support team works closely with each individual to create tailored support plans that reflect their aspirations and encourage greater choice, control, and community involvement. We also work collaboratively with multi-disciplinary teams (MDTs), health professionals, families, and other agencies to provide coordinated, high-quality support.

Whether someone is developing life skills, accessing education or employment opportunities, or preparing for greater independence, our focus is always on achieving positive, person-centred outcomes.
