Fundraising Activity Approval Form

Before fundraising activities take place, we ask services to complete this short form so the event can be reviewed and approved.

The purpose of this form is to ensure fundraising activities are safe, appropriate, financially viable, and well-planned, while supporting teams to deliver successful and enjoyable events for clients and colleagues.

This is not intended to create unnecessary barriers — it helps us provide the right support, manage risk proportionately, and ensure funds raised have the greatest possible impact.

Once submitted, the form will be reviewed by the Director of Finance, who may contact you if any clarification or support is needed.
